How to Apply:

Submit your completed form and all required documents within 60 days of your service date.

 We accept most insurance plans and offer flexible payment arrangements. Please speak with a team member before your scheduled procedure.

Payment Expectations:

• Co-pays and co-insurance are due on or before your procedure date.
   
• Depending on your insurance and deductible, you may owe a portion of your balance upfront.
   
• Monthly statements will be sent for up to three months or until your balance is paid in full.
   

For billing questions, please contact our billing department directly.

With multiple practitioners performing procedures at our facility, you may receive separate bills from your surgeon and the surgery center. We encourage you to:

1. Request a personalized estimate from each provider.
    
2. Confirm insurance participation, as some third-party providers may not accept the same insurance plans we do.
